Support for mothers, babies, and families

The Tuscarora Community Health Worker Program is part of the New York state Comprehensive Prenatal-Perinatal Service Network. The network reaches out to families in economically challenged areas that exhibit high infant mortality rates.

The purpose of Community Health Worker is to improve pregnancy outcomes, promote healthier mothers and babies and establish better linkages between existing health, social services and educational programs.

Ours is a community-based, family-centered program serving pregnant and parenting women on the Tuscarora Indian Reservation. We provide home-based case management services to participating women and their children and help them overcome barriers to care.

 Services include:

Identify complicating factors that make it difficult for clients to access the care they need

Assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus

N

Assist in enrolling pregnant women into prenatal care and children into primary care

N

Provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence and substance abuse

N

Most importantly, provide support and help to overcome the barriers that prevent families from receiving a healthy start in life
