Health Home
A Health Home is a community care management program for Medicaid members with chronic medical and behavioral conditions. At the Niagara Falls Memorial Medical Center Health Home, care is coordinated by a dedicated case manager who helps participants navigate the healthcare system and coordinates referrals to other services.
The Health Home program is free for qualified Medicaid patients.
What can the Health Home do for you?
- Help you find the doctors you need
- Help you make appointments
- Assist you with community resources such as housing, food, transportation, heat and legal services
- Provide health education
- Help link your family members with support services
Health Home Services
- Comprehensive Care Management
- Care coordination and health promotion
- Comprehensive transitional care (e.g. inpatient discharge to home)
- Patient and family support
- Referral to housing, legal, food and other community and social support services
- Linkage between service providers via a health information system
