Community Service Plan and Community Health Needs Assessment

Niagara Falls Memorial Medical Center’s 2014 Community Health Needs Assessment and 2014-2017 Community Service Plan include an assessment of public health priorities and Memorial’s strategies for addressing several priorities identified in conjunction with the New York State Department of Health’s Prevention Agenda.

The Prevention Agenda is a blueprint for state and local community action to improve the health of New Yorkers. 

The medical center’s priorities were chosen as part of a collaborative process involving the Niagara County Health Department and the other hospitals in the county.
