Home > Community Health Worker Program Community Health Worker Program The purpose of Community Health Worker is to improve pregnancy outcomes, promote healthier mothers and babies, and establish better linkages between existing health, social services and educational programs. Ours is a community-based, family-centered program serving pregnant and parenting women in Niagara Falls and on the Tuscarora Indian Reservation. We provide home-based case management services to participating women and their children and assistance in overcoming barriers to care.
